Description Summary: High Reliability Organization, the Coordinator Sepsis, reporting to the Director of Quality, is responsible for coordination activities of the Sepsis Clinical Improvement Program in the hospital. The Sepsis Coordinator works collaboratively with hospital and medical staff leadership to utilize data and evidence-based practice to drive strategies in performance improvement, regulatory, and patient safety activities that promote a culture of learning, improvement, and patient safety of the sepsis patient. Analyzes and reviews provided data for opportunities for improvement/process improvement to reduce the impact of sepsis and severe sepsis. This role is responsible for organizing and coordinating specific quality activities and for compiling/displaying/evaluating information from program for assigned hospital and medical staff committees and teams. This role is expected to apply clinical knowledge and analytical skills to assist the Director of QM and support change with a strong focus on improving quality outcomes and results. Coordinates activities of the Sepsis clinical program. Verifies that patients' needs are met and evaluates program performance. Participates in planning of work assignments and coordinates activities with other patient care units and related departments. Plans and organizes training for unit staff members and participates in guidance and educational programs. Domain Level: Foundational. Responsible for the day-to-day coordination of the department operations to ensure efficient use of resources in the delivery of patient care and staff scheduling. Assists Director of Quality with oversight of staff competencies, training, and quality assurance. Review of patient records to ensure accurate and complete nursing notes and required charting elements. Review of daily charge entry to ensure accurate and complete charging of nursing infusion procedures. Assist Director in identifying quality improvement opportunities and supporting quality assurance initiatives, in addition to coordination of data collection efforts. The Sepsis Coordinator is responsible for direct patient care, accreditation achievement and maintenance. Direct patient care responsibilities include the delivery of patient care through the process of assessment, diagnosis, planning, implementation and evaluation. In addition, the Sepsis Coordinator prescribes, delegates, coordinates, supervises, and evaluates patient teaching and the activities of the ancillary nursing personnel while maintaining the standards of professional nursing. The Registered Nurse is able to provide direct and indirect care to all patients, and provide leadership, coordination of care, data collection and management of the Sepsis patients. Requirements: Associate degree in Nursing required. Bachelor's degree preferred. 3 years of healthcare experience required. 2 years of quality management experience preferred. CPHQ (Certified Professional in Healthcare Quality) preferred.
